Advance care planning: Whose conversation is it anyway?

In this webinar, you will learn how to have conversations about the five steps of advance care planning (ACP) and how you can help people choose their substitute decision maker. ACP is the process of reflecting on and communicating one’s health and personal care wishes, including end-of-life care. Evidence shows that people who engage in ACP are more likely to have their wishes known and followed at the end of life. We will explore the nurse’s role in engaging in ACP conversations with patients and families as well as promoting ACP at a community-wide level. You will also learn about the Speak Up campaign and the five steps of ACP — think, learn, decide, talk and record.
